The proposed Workshop will bring together participants from the Virtual Institute ``Dense Hadronic Matter and QCD Phase Transition'' with representatives of the Initiative for a European Research and Training Network and on the ``Physics and Astrophysics of Neutron Stars''. We would like to promote discussions and collaborations among people working in Astrophysics, Nuclear Physics and Particle Physics, including both observers and theorists. The focus will be on the theoretical aspects and we will ask a few observers to explain the status and perspectives of recent experimental results.

The meeting will center on focused plenary sessions in the key areas in the morning that are supported by informal working groups with contributed presentations in the afternoons. The final day of the workshop will be dedicated to the presentation of summaries by the conveners of the topical working groups, and discussion of future networking in international collaborations.

The workshop will be divided into several sessions and working groups which are listed below:

• Physics of Dense and Super-Dense Matter ;
• Supernova and Neutrino Propagation Models ;
• Gravitational waves from Compact objects ;
• Observations with Neutrino, Gamma-ray, X-ray and Optical Observatories.
